and banding. Forgot to record weight at last catch up but I remember his BCS was 2.5

will need to discuss if we want to retest.

A: BAR. 110 flies with shoulders hunched/scrunched up slightly, but they can get

where they almost touch the wire, if they're not touching.

Cleaned carpal wound with dilute chlorohex, did not redress wound. Otherwise, wing seems to have good extension and callusing well. Evisceration site looks good, no swelling, drainage, or dehiscence. Administered PO med. Found one cast and zero scraps.

R wing stretches easily to full extension, no crepitus or shifting felt. Sutures on metacarpals still present. Bird does not react painfully to manipulation of this wing. Stable callous formation palpable.

S: BAR when caught up for surgery.

O: See surgery report for details. Evisceration performed OD. Bird crashed during procedure, revived with one round of atropine and epinephrine, large amount of bleeding during surgery after epinephrine given. Recovered uneventfully from anesthesia, although a post anesthesia heart murmur was noted during recovery.

A: OD eviscerated. Bird unstable for part of procedure but seemed to recover without issue.

P: Monitor evisceration site for dehiscence, drainage, swelling, bruising.

Performed by - AD
Surgery time - 30 minutes

Pre-medicated with torb IM, then anesthestized via isoflurane
facemask.  Intubated with 3.5 uncuffed ET tube.  Bird placed
in dorsal recumbency, R eyelids plucked ans scrubbed for surgery.
Made a stab incision into the cornea, then cut out the cornea
at the scleral junction wth curved iris scissors.  Removed luxated
lens and intraocular contents.  Bird developed sudden bradycardia,
heart sounds very quiet, given atropine and epinephrine IM. 
Patient recovered and remained stable for rest of procedure.
 Removed nictatating membrane and eyelids via sharp dissection.
 Heavy bleeding from the back of the eye and from where the third
eyelids and eyelids were removed, held pressure and used curapson
until hemastasis was achieved.  Flushed the back of the eye with
saline.  Closed the eyelids with 5-0 webcryl in a simple continous
pattern.  Bird recovered uneventfully from anethesia, but a post
anesthesia heart murmur was noted during recovery.

A: Metacarpal fracture has rebroken slightly after IM pin was pulled, but carpus feels better without IM pin in place limiting extension of the carpus. May still heal well enough for good flight, will need more time in wrap to fully stabilize.

P: Monitor fabric wrap, change every other day with PT. Plan for evisceration surgery this week for OD with lens luxation.

A: Better alignment and stability with pin but due to multiple fractures and inability to safely place cross pins bird will need to keep figure-8 wrap on wing.

Surgery report
Procedure - Fracture repair - metacarpus
Performed by - AD
Surgery time - 40 minutes

Pre-medicated with torb IM, anesthetized via isoflurane facemask,
then intubated with 3.5 uncuffed ET tube.  Plucked and scrubbed
the R metacarpus.  Placed bird in dorsal recumbency and incised
over the fracture site with a #15 blade.  Bluntly dissected down
to the fracture site, placed IM pin retrograde out the bent carpus,
blunted the other end of the pin, lined fragments up and drove
IM pin across the fracture site to seat at the distal end of
the metacarpus.  Took intraop x-rays to confirm placement. Exposed
bone along the alular digit.  Debrided sharp, exposed bone end,
cleaned and closed skin over end of bone with 4-0 webcryl simple
interrupted sutures.  Flushed incision site, closed with 4-0
webcryl in a simple continous pattern.  Cleaned small distal
metacarpal wound, closed with sutures.  Turned bird onto ventral
recombency, cleaned carpal wounds and small wound on dorsal aspect
of metacarpus.  Closed these wounds with single interrupted sutures.
 Bent IM pin around the dorsal aspect of the metacarpus, taped
end.  Placed TAO, telfa/tegaderm over sutured area, placed tegderm
on IM pin to keep it down.  Placed figure-8 wrap on wing.  Patient
recovered slowly but uneventfully from anesthesia.

O: Figure-8 and splint in place on R wing, left alone. Small/superficial scabs on a few toes. Scab on R dorsal hip, removed and found healed skin underneath. Scar tissue and missing feathers present over the mid-distal L patagial tendon, wing has good extension. Very small amount of unabsorbed fluids over the caudal coelom. Large amount of green bruising in AD. Checked eyes (see eye exam). MM tacky, small bruise seen at back of mouth. Drew blood from the R medial metatarsal vein.

A: R metacarpal fracture need surgery ASAP, prognosis still guarded due to presence of midshaft fracture and more proximal fracture. Both eyes also potentially compromised, although bird is not acting like a blind bird in cage or in hand currently.

P: Switch to pred drops OU to try to hit eyes hard with anti-inflammatories. Plan for surgery today to try to repair metacarpal.

Gave .13 simbadol IM in L keel and .79 dex in R keel. Gave 30cc LRS SQ in R inguinal area. Gave .79 Ca slurry and .24 pentox PO. Sprayed with adams.

A: BAOW hit by car with a R metacarpal fx and severe ocular trauma. Prognosis is very guarded, as R metacarpal fx may not be fixable via surgery, OD likely blind, and OS may not provide enough vision for release

P: Schedule surgery to repair metacarpal fx for 3/14. Rx Ca slurry, pentox, tramadol, Clava, Baytril, keto OD, LRS SQ SID through 3/16, and meloxicam starting 3/14 PM. Take IOPs 3/14. Monitor R figure 8 wrap. Monitor bruising AU. Perform initial exam on back/ near uropygial gland (missed during intake exam).
